New Haven Tutoring Initiative Registration

I understand that my child's after school program/school was awarded a grant to integrate and promote reading or math into their current program. As a result my child will be participating in the New Haven Tutoring Initiative as part of their regular after school program/after school.

Regular attendance is required. Please have your child attend for the full hour, twice a week. If they are ill or unable to attend due to an unforeseen circumstance please contact the site director to inform them prior to the session.

I understand that NHTI might use photographs and other recordings of my child's likeness, voice and sounds, and reuse or license the right to reuse such photographs and recordings of his/her name, likeness and biography in all media and in all forms, including, but not limited to, his/her participation in NHTI, without compensation to me or any limitation whatsoever.

I agree to have my child assessed during the year to determine placement and measure growth.

I agree to have my child's data shared with NHPS in order to determine impact and effectiveness of the high-dosage tutoring my child is receiving at NHTI.

Regular Attendance: I commit to ensuring my child attends program sessions regularly and consistently, understanding that attendance is crucial for academic progress.

Punctual Drop-off & Pick-up: I commit to dropping off and picking up my child on time, respecting program schedules and staff time.

Home Learning Support: I commit to supporting my child's learning at home by encouraging practice, helping with homework when appropriate, and creating a positive learning environment.

Open Communication: I commit to maintaining regular, respectful communication with program staff and responding promptly to messages or concerns.

Positive Attitude: I commit to maintaining a positive and supportive attitude about the program, encouraging my child's participation and celebrating their progress.

Necessary Supplies: I commit to ensuring my child comes prepared with any required materials, supplies, or equipment as requested by program staff.

Respectful Interactions: I commit to treating all staff, volunteers, other families, and students with respect and kindness in all interactions.

Prompt Notifications: I commit to notifying program staff as soon as possible about any absences, changes in circumstances, or concerns that may affect my child's participation.
